The Verification/Authorization by Agency form must be completed and submitted.
SOP Cardiac Self-Assessment This must be completed and turned in, prior to written testing.

SOP Fundamentals Self-Assessment This must be completed and turned in, prior to written testing. The full SOP can be found under the Standards of Practice tab.
SOP Medical Self-Assessment This must be completed and turned in, prior to written testing. The full SOP can be found under the Standards of Practice tab.
SOP Trauma Self-Assessment This must be completed and turned in, prior to written testing. The full SOP can be found under the Standards of Practice tab.
ECG Study Guide The ECG study questions do not have to be completed prior to written testing. However, it is strongly suggested that you review the guide prior to taking the ECG strip test.
System Entry testing is done on the Second and Third (2nd & 3rd) Tuesdays of the month. Please contact the EMS Office to Schedule.
After successfully completing written testing, skills testing is next. Please select the Preparations for Skills Lab tab.
