A Better Way to locate Forms From IDPH EMS

 We, as a system are trying make sure the system members always use the most current state forms when requesting action from IDPH.  This can be challenging, as there are a number of forms and users do not always know when a form is updated.

Under the EMS System's  policy manual, whenever a from is a part of the policy we will try to have the most current version posted.  It is just as, or even easier to use the new link found under the Standards of Practice tab on the home page.  It is called IDPH Forms & Info.

You will be directed to the EMS Section of the IDPH website and ther you can scroll down to whichever form you are seeking.  It can then be downloaded, or printed, depending on your needs.
