The attached form will give us the meaningful data we will need to determine if a change in protocol is necessary.
We are asking that they fill one out on every patient in cardiac arrest on whom resuscitation is initiated and forward to you.
· When they are received by the PEMSC, please scan and forward to Dr. Jordan (mjordan@nch.org ) within one week of receipt so we are watching real-time data evolve.
