Important Announcement regarding Annual CE Hours From Connie Mattera

Last year, I was asked to create a one page form that would help tally up all the CE and mandatory requirements for the year that could be used to provide feedback to individuals who may be complete or incomplete when the EMS Coordinators/Educators do their annual review of the files. This really is just a different way of looking at the one page CE forms that you already have in the binders at the agencies, but is something that can be printed and given to the individual.
Use of this form is purely optional, but attached is this year's edition if you would find it helpful. Please note that we are reducing the number of CE hours needed this year owing to the fact that April was an agency-conducted class that did not carry CE credit.
As in the past, the nurse EMSC/educator must also see a current license for all paramedics.
Due date: The CE hours, CPR cards, 4 intubations, Infection Control Education, and annual restraint competencies must be complete and tallied by June 30th or intent to suspend notices will be sent to the individuals and their agencies. If the requirements are not met by July 12th, suspension notices will be sent electronically that will go into effect on July 15th. I would trust that this contingency would not be needed.
