The Illinois Department of Public Health is charging a fee for initial and relicensing of all EMS licenses.
To change your address with the Department of Public Health, click on the link for Online Services.
To pay your license fee with the Department of Public Health, which you must do before you can receive a license, click the link for Online Services.
For IDPH Forms and Documents, please click on this link to take you to the IDPH website. Click on Forms and Documents on the top of the page and then, Scoll down to the EMS Section of their page for the most up-to-date information:
If requested for the sytem's ID number on the online renewal form - it is 0907
